Assists older adults to remain as independent as possible, through programs and services such as care management, disability resource center, Medicaid Waiver, congregate and home-delivered meals, legal assistance and in-home services. Caregiver support, senior health insurance information program (SHIP). Eligibility based on age -60 or older- but other considerations may be given.
